Q. Hi, I had rhinoplasty at the end of 2004 and that was mainly to make my nose look narrower by reducing the nostril . However, as a result, my nose tip is now slightly more tipped upwards and the tip itself, although somewhat narrower still is not how i want it. Is this something you could help with and if so what are chances of getting successful result this time. Kind regards,
A. Thank you for your inquiry. It is possible to change the direction of the tip either up or down as well as contouring it differently. The most important element to begin with is to have a clear plan for what she would like to achieve. For this reason, I typically use computer imaging and stimulation during the consultation in order to clarify your goals and what is realistic to expect from surgery. It is possible to achieve changes along the lines of what you wish to see, but you need to bear in mind that the outcome of surgery is never identical to the computer simulation.

Overall, after 9-12 months from the original intervention and it is possible to proceed with a revision, as the tissue mostly around the tip of the nose has had a chance to mature. I do suggest however that he discuss your concerns with the consultant and has been responsible for your care, as I am sure he or she would want to remedy the situation.

Q. hello i was wondering if the use of alloderm is common in th uk for thinned skinned patients. I have had a couple of rhinoplasties and can clearly see indentations and irregularties on the front profile of my nose and clearly feel them. On my front profile it looks like i have an inverted V engraved into my nose. what is this and is it hard to fix? Are grafts successful in camoflaguing with thin skinned patients?,
A. Thank you for your inquiry. The most common treatment for the inverted V deformity that you describe, is repeated in fractures and spreader grafts along the dorsal septum. For camouflage, I do not tend to use AlloDerm, but I would be happy to use Permacol if needed. Alternatives that involve your own tissues include temporal fascia graft or crushed cartilage graft. An additional treatment option in this situation like yours, is a small amount of fat transfer to the bridge of the nose, although this is not the most common approach. The aim of the surgery however would be to correct the visible rather than palpable irregularities.
