Aged 29 I had rhinoplasty about 4 years ago for a large nose with a diamond fracture

Q. Aged 29 I had rhinoplasty about 4 years ago for a large nose with a diamond fracture, excessive cartiledge in the tip and it pulled to the right when I smiled. Ever since being done I have small bump on the left of bridge which isn’t visible to the eye but is to the touch and sometimes the tip looks bigger than the bones connecting in to it and can appear a bit bulbous at the end. I had an open rhinoplasty and when I smile its like the skin between my nostrils hasn’t settled properly and it can look a bit lop sided. I had to massage a lump of scar tissue on the inside of my nose for months to get the scar to settle. I went back to the original company that did the op but the surgeon had left so I saw another. He said I should be happy with the result and if I wanted to change it then it would mean another open rhinoplasty which I found off putting. I’m after a second opinion really about whether I should consider secondary rhinoplasty or just to be happy with the result. Thanks,
 
A. Thank you for your enquiry. This is a type of question that can only be answered properly after a consultation. If the problem that you are faced with is a relatively minor one, it may be more practical to accept the result for what it is. If you find that it constantly makes you think about it and does not give you peace, there may be some scope to have a consultation using the imaging in 2-D or in 3-D, to identify your target for change. One thing that is very important though is to appreciate that movement of the facial muscles distorts the nose in everybody, and there is very little value in pursuing surgery in the hope that the nose will stay the same when you smile.

Q. I had rhinoplasty in 2007 to correct a hump on the bridge of my nose – the surgeon told me that I should have the tip lifted also, despite the fact I was fine with the way it looked before. At first things looked great but after the swelling went down, the tip of my nose has a huge cleff in it (one side of the cleff sticks out further than the other) and from the front in photos it looks like the bridge has been smashed, there is no real definition to it – it’s very wide and the hump seems to have been moved off to one side of my nose rather than removed. At my follow up appointment the surgeon told me after the surgery that I had a ‘difficult nose’ and he would not feel confident about correcting it. Is there anything that can be done? As this is would be revision rhinoplasty I really want to get it right – I also worry about the cost.,
 
A. Thank you for your question. Whilst it is possible to to improve on the appearance of the nose, one of the critical aspects at this stage is some accurate planning regarding what you would like to see as an end result. In some instances also, smaller adjustments, sometimes nonsurgical can bring sufficient benefits for patients to feel comfortable. With this in mind, I would suggest a consultation using computer based simulation either on standard photographs or 3-D imaging, to help you determine what it is you would like to achieve and communicate it clearly with me. It would be possible then to give you guidance regarding how much change can be operated safely, and what type of intervention may be best suited for the purpose. It is also advisable to see more than one surgeon to allow you to get the overview regarding the current situation and options for change.

My friends and family all say i am nuts and that there is nothing wrong with my nose but i am obsessed with it

Q. My friends and family all say i am nuts and that there is nothing wrong with my nose but i am obsessed with it. I feel it is way too big for my face and want it making smaller. Is this possible to make somebodys nose smaller. It is not bent or hooked or anything like that just too big?,

I have a one year aftercare policy and if you’re unhappy then you need to tell me within a year for me to do further surgery without charge. In addition, expect them to have the right qualifications and training either through the channels of plastic surgery or ENT, but most importantly meet several surgeons so you can get a clearer idea of their approach and how it corresponds to your designs for change. I tend to favour using computer imaging and procedure planning during the consultation, but every surgeon will have their own approach. Finally, you can also ask your surgeon if they can put you in touch with previous patients that had surgery with him.

A. Hi – I have a clinic in Glasgow and my price for a non surgical rhinoplasty is £300 – £200 for the treatment and £100 for the consultation. You can have treatment on the same day as your consultation but it will have to be in Glasgow.
