Q. I was born 26weeks premature and my nose is different both sides i was wondering how could i get it to be the same shape both sides ?,
 
A. Thank you for your enquiry. First, I would like to mention that everybody has asymmetry of the nose, and this is the very reason why actors and actresses refer to their good profile and bad profile. A substantial degree of asymmetry becomes more readily noticeable, and surgery can reduce the contrast between the two sides and improves the overall balance.

Q. Hi Mr Lucian Ion, I have had two nose jobs in total, and it has been 8 months post op now, however unfortunately I am still unhappy with the results (infact I dislike the 2nd nose job more than the 1st). I want to go back to the clinic to enquire about having further surgery to amend the parts I dislike. I feel that as my nose looks puggy with alot of nostril on show and the comuella looks hanging to me and I just don’t think it suits my face as it is. I’m obviously very concerned about having more surgery incase it looks even worse again and the nurses told me that a good surgeon would not carry out more than 2 operations in case my nose collapsed. But I am still truly unhappy with my nose. Do you have any advise for me. Is there anyway I can e-mail you some photos of my nose for your opinion? Many thanks, Tracey,
 
A. Thank you for your enquiry. The information you have been offered regarding the number of operations that is possible for nasal reshaping is inaccurate. The risk of the nose being distorted after surgery depends on the surgical technique, the support provided or the possibility of an infection, rather than the number of surgical interventions carried out in the past. I would be happy to evaluate the situation, and the best would be to discuss it in a formal consultation, but you have to bear in mind that you need at least 12 months from the second operation before you have more surgery, to allow the tissues to become softer and for the scar inside the nose to mature more.

Q. Q. Hello there. I seem to have little, if no, cartilage in the tip of my nose, which for years I’ve been considering having slightly reduced / reshaped. Q. I broke my nose at 11. I knew it was broken. I could bend it side-to-side. My nose is wonky, and i hate it! I cant look at myself in the mirror and i avoid pictures. Should I get a nose job? Can i have it straight and a different shape?,

Q. Have you done many ethic rhinoplasty before? I am considering having some work to adjust my nasal tip as it is sort of droopy. Would that involve taking a cartilage from the rib or using an implant? Thanks,

Approximately 10% of the rhinoplasty work that I carry out is ethnic rhinoplasty, typically involving rib cartilage grafts for support of the nasal tip and enhancement of the nasal bridge. I am well aware that silicone implants are commonly used for ethnic rhinoplasty interventions, but I feel that when more significant definition is requested by the patient, I prefer to rely on a stronger material.

Interestingly, significant reduction in the size of the nose is a relatively common request, and the algorithms that have been developed allow for a significant amount of correction. With a large reduction I typically use the open approach, and most often and alar base reduction is also included. The recovery period involves approximately 2 weeks for return to work for most people. Q. Please can you tell me how much an initial rhinoplasty consultation is, what is the length of time to receive an appointment and how long is it for? After the consultation, how soon would you normally be able to fit someone in for the rhinoplasty procedure, eg is it within days, weeks? I suffer from hayfever, thankfully it hasn’t started yet. How will this affect your decision to perform the procedure. I normally take Beconase Hayfever and loratidine. With regard to taking time off work after the procedure, what would you normally recommend?,
 
A. Thank you for your enquiry. It usually takes a few weeks between an enquiry and the consultation, and the time interval depends also on your availability and flexibility for the attendance. The appointment typically is for 45 min for a rhinoplasty first consultation. Typically, between consultation and surgery a period of 3 to 6 weeks is necessary, with a minimum of two weeks being anyway the recommended cooling off period. It appears that hayfever has relatively little influence on the after-care for rhinoplasty, since for the first week I normally leave internal silicone splints, which prevent the contact of the allergens and the nasal lining. I would recommend however using sinus rinse salt water irrigation is for the nose, which are part of the after-care, also in the pre-operative period for hayfever sufferers. I recommend taking two weeks off work for this intervention, essentially in relation to the bruising and early swelling. Typically, the appearance is relatively good at about 10 days, but fluctuations in the amount of bruising and swelling are common, and it is better to have a margin of annual leave.
