I had rhinoplasty a couple of months ago and even though i am much happier with my new nose

Q. Hi, I had rhinoplasty a couple of months ago and even though i am much happier with my new nose, however i still do not like the profile as it still looks very big to me and not much different to my old nose. I understand you have to wait up to a year before you can do anything but can this be changed so my profile is smaller?,

Can you advise and recommend a surgeon?,
 
A. Thank you very much for your enquiry, it’s certainly does sound as if you have had slightly reduction in the bridge of your nose and dermal fillers is certainly an option for you. Many plastic surgeons do perform dermal filler injections to the nose and you’ll be able to find a list of surgeon by looking at the BAAPS website. I had an open rhinoplasty 9 months ago and I am very unhappy with the results

Hello, I had an open rhinoplasty 9 months ago and I am very unhappy with the results and I am definately considering a revision. i know that this kind of procedure is very diffeicult and require specific expertise. I was wondering if you would be able to advise me on finding the best surgeon for the job.,
 
A. Thank you for your question. I think at this stage it is very important that you try and build a very clear idea of what you would like to see as an end result. This can be assisted during the consultation by computer imaging, and clarity in terms of the surgical plan is very relevant for the end result. In terms of finding the surgeon, I suggest that you try and see several people, in particular if from their websites you have found that the pattern of work they do corresponds to your general direction for change. There is no hard and fast rule regarding finding a surgeon most suitable for you, but trying to contact and discuss your problems with several people gives you a broader appreciation of the subject and approaches. In addition, you can always ask if you can discuss with patients that had surgery with the same person, if possible with a reasonably similar scenario.

I want to enquire whether you do secondary rhino plastry

Q. I want to enquire whether you do secondary rhino plastry this is a query for a friend and also i broke my nose as a child i did have surgery but still have a bump can this be corrected by a filler.what are your dates for february for sculpta clinic or do we need to come to la belle form thankyou.,
 
A. Hi there. You need to come and see me at La Belle Forme, and I’m afraid surgery dates for February are now full, but I can still see you for a consultation in February. If you’re just having a filler rhinoplasty then I can that treatment at the time in the clinic for you. A bump can be smoothed off and improved, usually, by a filler, but sometimes you do need to have a full rhinoplasty rather than a filler rhinoplasty. I do perform secondary rhinoplasty and it’s important to accept some of the limitations of surgery when we’re doing secondary rhinoplasty, as it’s a more unpredictable procedure unfortunately. However, it is something I can do for many people.

Is it possible to make subtle amendments, (particularly correcting the tip of the nose which point up and my nostrils looked flared? A. Thank you for your enquiry. Open rhinoplasty’s do take longer to settle down than a close rhinoplasty’s as a residual swelling and numbness of the skin of the nose plus can persist for up to 6 to 8 months after an open rhinoplasty. You say that it is now 2 years since you had an open rhinoplasty so I’m quite sure that the effects of the open rhinoplasty would have settle down by now.Subtle adjustments can be made to the tip of the nose but this again would need open rhinoplasty as you need to be very clear in visualization of the adjustments that need to be done on the tip of the nose .This may need ex channels surgery in the form of alar base reduction that you would entail putting a scar on the base of the tip of the nose. You would need a detailed consultation with a plastic surgeon who is undertaking rhinoplasty and regular bases. The surgeon has to contend with the effects of a scarring from previous surgery as well as unsatisfied patient who is not happy with the results of the primary rhinoplasty. It is absolutely essential that you have a detailed consultation with a surgeon, other plastic surgeon or ENT surgeon who is qualified in udertaking primary and secondary rhinoplasty’s. Yes I do perform revision rhinoplasty regularly and would on an average I do 5- 8 revision rhinoplasty’s every year.I would say that revision rhinoplasty is a successful procedure but it would depend on what the patient expects to achieve after a secondary rhinoplasty and it is for this reason that a detailed consultation is absolutely essential. I had rhinoplasty through Transform in 2007 to correct a hump on the bridge of my nose

Q. I had rhinoplasty in 2007 to correct a hump on the bridge of my nose – the surgeon told me that I should have the tip lifted also, despite the fact I was fine with the way it looked before. At first things looked great but after the swelling went down, the tip of my nose has a huge cleff in it (one side of the cleff sticks out further than the other) and from the front in photos it looks like the bridge has been smashed, there is no real definition to it – it’s very wide and the hump seems to have been moved off to one side of my nose rather than removed. At my follow up appointment the surgeon told me after the surgery that I had a ‘difficult nose’ and he would not feel confident about correcting it. Is there anything that can be done? As this is would be revision rhinoplasty I really want to get it right – I also worry about the cost.,
 
A. Thank you for your question. Whilst it is possible to to improve on the appearance of the nose, one of the critical aspects at this stage is some accurate planning regarding what you would like to see as an end result. In some instances also, smaller adjustments, sometimes nonsurgical can bring sufficient benefits for patients to feel comfortable. With this in mind, I would suggest a consultation using computer based simulation either on standard photographs or 3-D imaging, to help you determine what it is you would like to achieve and communicate it clearly with me. It would be possible then to give you guidance regarding how much change can be operated safely, and what type of intervention may be best suited for the purpose. It is also advisable to see more than one surgeon to allow you to get the overview regarding the current situation and options for change.

I would like the end of my nose reshaped but would rather not have surgery to fix the problem

Q. I would like the end of my nose reshaped but would rather not have surgery to fix the problem and wondered if there was a non surgical solution that could work for me. If there is would the solution be permanent or would I be required to return for further treatments?,
 
A. There are some non surgical solutions for some nasal changes we want to see but they don’t work for every change that people are looking for. Non surgical solutions are temporary I’m afraid, but they’re usually very straightforward treatments and take a 30 minute appointment to perform.
