I visited my GP recently as in the summ i fell up the stair and banged my nose

Q. I visited my GP recently as in the summ i fell up the stair and banged my nose, since this there has been a dull pain to it & it slighlty effects by breathing… GP felt it & said that there is some old scarring there but said there was nothing that could be done about it from her side! I also have a small lump to the right hand side of my nose – which i’v had since my realy 20s…. is there anything that can be done about it? I hate to look in the mirror & feel conscious when talking to other people that they are looking at my nose. Every one else thinks there is nothing wrong with it, but I am unhappy about it.,

A. Thank you for your enquiry. In a situation like this, and a consultation using computerised imaging and planning either on standard photographs or in 3-D, can clarify whether there is justification for surgical intervention. In some instances, body dysmorphic disorder is the source of this unhappiness about the appearance of part of the body, and surgery is not likely to give you peace of mind. With regards to the pain, chronic pain after trauma or surgery is well recognised. It is not usually easy to treat, and some instances can become worse after more intervention to the area, such as with surgery.

I had a rhinoplasty many years ago, but have had problems ever since

Q. I had a rhinoplasty many years ago, but have had problems ever since. My noseis noticeably crooked, and one nostril is completely blocked. I often suffer from sore throats and suspect there’s a connection. Could revision nose surgery possibly help my problem?,

A. Thank you for your enquiry. Overall, the revision nose surgery can address the shape and the nasal airway. It is difficult however to be sure whether your sore throat problems is a direct consequence of the nasal airway difficulties. It may also be related to tonsil problems, and it would be a good idea to discuss this with an ENT specialist.

My nose has been through a lot of stress as it was broken as child

Q. My nose has been through a lot of stress as it was broken as child and fixed twice then a few years after suffered valve collapase. During a visit to the doctors I told him about some breathing issues I had been having and he advise I MAY have a septum which deviates to the left but he cannot be sure as he is not a nose surgeon and suggested me seeing one. My nose is slightly bent and I wondered if more surgery would be a good idea to an already fragile nose. From information I have read the more surgery I have on the nose the more likely the complications after each procedure. Im very concerned about the breathing problem but I am prepared to puit up with it if it means I am not putting my nose in harms way once more. Thanks for any help you can offer. Pamela,

A. Thank you very much for your enquiry. I really would suggest that you see a nose surgeon and in fact an ENT surgeon would be the best person to advise you. The ear, nose and throat (ENT) surgeons specialize in breathing problems inside the nose and would be able to examine your nose internally to see what is causing the breathing problem. They also have a lot of expertise with reshaping the nose, which may be appropriate in your case. I wish you the best of luck with your consultation. I’m sure they’ll be able to help you decide the best way forward for you. Best Wishes, Yours sincerely Prashant Govilkar Plastic and Cosmetic Surgeon Consultant, Aurora Clinics

I have some difficulty breathing through one of my nostrils and think this may be due to a deviated septum

Q. I have some difficulty breathing through one of my nostrils and think this may be due to a deviated septum. I also have a ‘bump’ on the top of my nose and was wondering what procedure I would need if I wanted to tackle both of these issues at the same time and how much this would cost.,

A. It sounds like you’d need a humpreduction, with infracture and a septal correction, ie a septorhinoplasty Best wishes Barbara see www.barbarajemec.com for information on procedures, prices and consultations

I have has three lots of sinus surgery using endoscopic technique to open passage ways of my nose

Q. I have has three lots of sinus surgery using endoscopic technique to open passage ways of my nose and also to remove some polyps to help my breathing problems which have been causing me to have headaches especially in the mornings when I wake up. Yet i am still as bad as ever and I think that the surgeon is missing the fact that I think i have deviated septum as well but he insists everything is fine yet 6 months after final op I still cannot breath properly. Is this something i can come and get second opinion as I feel any further surgery on sinus alone is not fixing problem and something is wrong with actual structure of nose. I have lost faith in ENT after this and now wonder if plastic surgeons are more likely to be able to help me. Many thanks for getting back to me,

A. HI there Unfortunately it sounds as though you have a functional problem and usually this is very much the domain of the ENT surgeon. Depending on examination of your airways and nose, it could be that a SMR is indicated too. This is an operation performed by both ENT surgeons and Plastic surgeons but is being performed less often by plastic surgeons due to referral pathways. Best wishes Paul Banwell
