Would restylane injections be suitable for a dent in nose after previous rhinoplasty surgery?

Q. Would restylane injections be suitable for a dent in nose after previous rhinoplasty surgery? I would rather not have to have any other operations fopr cost and well being purposes but if I can get something done about this I would be very happy indeed. Regards,
 
A. Thank you for your enquiry. Hyaluronic acid injections can help create a smoother contour after rhinoplasty, and they would usually last for approximately 6 months. Radiesse injections can last up to a year. Although permanent fillers can also be used for the purpose, I would be more worried about that alternative.

Im wondering if you could help me, over the past 6 or 7 years the cartilages in the tip of my nose has split and become very noticable

Im wondering if you could help me, over the past 6 or 7 years the cartilages in the tip of my nose has split and become very noticable.Im now left with what looks like a lump on the left side of my nose. i can actually twist it and hear it click back to normal.I experience numbness n pain every so often. I have not experienced any trauma to my nose so cant understand why it has done this. This has left me very self consious and is effecting me self essteem. im wondering if you could give me any insight to why this has happened and if surgery is my only option, thank you,
 
A. Thank you for your enquiry. The only thing I can assume based on your description, is that you may have had an injury to the nose in the past that has led to fracture of the cartilages, and after the swelling has decreased and the scar tissue has become softer, you have started to notice the changes. This can easily be several years down the line from the injury. It is however just an educated guess, and I would need to examine your nose in order to be able to give you some answers regarding management. Another factor that contributes, is that in time, the skin of the nose tip can become either thicker or thinner. If it becomes thinner, the underlying cartilages will show more.

Im 18, my nose sticks out quite far, bends slightly in a hooked fashion

Q. Im 18, my nose sticks out quite far, bends slightly in a hooked fashion, and is quite fat at the end, could these things be addressed in one procedure, and am I old enough to have it done?,
 
A. Thank you for your enquiry. The simple answer would be yes and yes. It is important however to approach this with good preparation. I would suggest that you involve your family with your decision, and with the planning for the changes. Although you are the only one that can judge what is good or bad for yourself in this matter, support from people around you is very helpful. I normally advise using computer imaging and planning for the intervention, to bring clarity to the decision and help establish an accurate plan for the changes. Whilst it is true that the outcome of surgery will never be identical to the digital blueprint, starting the changes with a clear plan makes me feel much more comfortable then relying on simple discussions.

How many rhinoplasties on average would you say a surgeon would perform a year to make them experts?

A. Thank you for your enquiry. Sadly, your question does not have a simple answer. If numbers were the only measure of quality, you should probably look towards the Far East or for a rhinoplasty expert, is the ratio between number of plastic surgeons and requests for rhinoplasty is far different from the Western countries. Number certainly mean exposure, but it is a matter of devotion and interest in the subject that combines with experience to make expertise. Someone who is really interested in rhinoplasty surgery, can probably achieve consistent high-quality results from something like 30 procedures a year, but I would imagine that a combination of dedication and higher numbers can achieve even better results. Finally, someone who is very busy, may wish to spend less time customising the surgery to your own needs, and sadly the fact that a rhinoplasty result may look good to everyone around you, does not automatically mean you would feel happy.

When having rhinoplasty revision surgery is it difficult to lower the tip and shorten the collumela at the same time?

Q. Im am considering having revision nose surgery but wanted to ask you something that has been bothering me for a while. When having rhinoplasty revision surgery is it difficult to lower the tip and shorten the collumela at the same time?,
 
A. Thank you for your enquiry. The changes that you are describing are compatible, but there is a matter of degree in terms of the nasal tip de-projection. The more the tip projection is reduced the the more difficult it is to achieve definition. Shortening the columella can be achieved both in terms of the view from below and the view from the side, and doing a computer simulation it is possible to define during the consultation what type of change you would like to operate.

A. Thank you for your enquiry. For professional singers and actors I recommend 3 to 4 weeks of recovery. It does depend on how complex the surgery is and indeed the recovery may be somewhat faster. You do have to bear in mind that there is a reported risk for long-term change in the tonality of the voice after rhinoplasty. I have not seen it to be a problem so far, but it is well documented is a risk in the medical literature.
