A. Hi there. We can sometimes improve things after a rhinoplasty with a revision rhinoplasty and with your enquiry it seems there are a few issues for us to consider. You have breathing problems, so I need to see your septum. If this is normal then we have a couple of things we can consider doing as well. For the tip, this is sometimes the hardest thing to improve, but we sometimes want to have a close look at the tip and either reshape, reposition or add/subtract cartilage to the tip to help with things. Costs for revision rhinoplasty are usually around £3700 and we would want to keep you in hospital until you’re well enough to go home. Sometimes that’s an overnight stay – we just don’t want you to go home and then come back in again immediately, so we do like to check things are fine before you’re discharged. I would suggest that you involve your family with your decision, and with the planning for the changes. Although you are the only one that can judge what is good or bad for yourself in this matter, support from people around you is very helpful. I normally advise using computer imaging and planning for the intervention, to bring clarity to the decision and help establish an accurate plan for the changes. Whilst it is true that the outcome of surgery will never be identical to the digital blueprint, starting the changes with a clear plan makes me feel much more comfortable then relying on simple discussions.

A. Thank you for your question. It does depend on the material that was injected, which most certainly was not botulinum toxin. Some of the products are very difficult to clear, and may require an open-tip rhinoplasty to be able to clear the material. The first thing to do is to find out from the practitioner that did your treatment what it is that they have injected, and whether they can remove it for you. Based on this information, it is possible to plan for some treatment options. Unfortunately, permanent injectable fillers are often difficult to clear completely if they run into difficulties.

I’m afraid I don’t do minor changes to a nose, because the swelling and bruising after a rhinoplasty completely negate any subtle changes performed intraoperatively, and we still see the final results of a rhinoplasty after a year, no matter whether it’s a mini rhinoplasty or a complex rhinoplasty. That’s not to say we don’t perform precision, accurate and small procedures in a rhinoplasty, it’s just that many people associate mini procedures with minimal recovery times, and that’s simply not the case with work on multiple areas. I get the impression that what you you want isn’t a mini rhinoplasty; what you want is small changes but in many areas of the nose, and people who have changes to many areas of the nose have a lot of bruising and swelling after surgery because we’re operating on many areas.
