Q. mr lucian ion thanks for answering all my questions as its just been 7 months and few days i had open septo rhinoplasty where my dorsum and my tip is reduced way too much and my tip is turned upwards i want to ask you how long should i wait before i book an appointment with you and reading your answers make me feel everything can be fixed many thanks,
A. Thank you for your enquiry. It is possible to proceed to a consultation at this stage, which would give you more time to prepare for the surgery. The surgery however is best deferred until 9 to 12 months following the previous intervention. At the time of the consultation it is possible to define the changes that you would like to achieve with the assistance of computer simulation.

Q. I am 13 years old and I broke my nose last year, what is the minimum age I would be able to undergo rhinoplasty surgery with parental consent?,
A. Thank you for your enquiry. In general terms and, I preferred to undertake rhinoplasty after the age of 18, when the facial features enter a somewhat slower phase in development. In the context of post-traumatic changes, in particular if quite significant, I would be happy to evaluate the situation from around the age of 17, so the nasal development is relatively complete.

Q. Dear Sir. Id really like to find a soluition to my snoring problems. This is causing great problems at home and I although I have sort help from the doctor and he has assisted me in the short term he said the best solution would be to see a plastic surgeon.,
A. Thank you for your enquiry. Snoring is more frequently dealt with by ENT surgeons. I would recommend Mr William Grant, who is based at the Cromwell Hospital in London, on the basis that I am familiar with his work. I’m convinced there are many specialists across the UK who can deliver high-quality services, but being a plastic surgeon I’m not so familiar with the ENT group of specialists. Plastic surgeons can contribute improvement in some instances of snoring, by means of septo-rhinoplasty interventions, if the cause of snoring is primarily an event related obstructed nasal airway.

Q. Hi, I have previously had phinoplasty 15 years ago and revision 5 yrs ago, in a nutshell i have been left with what looks like a ski slope due to to much bridge taken of, therefore was looking into non surgical nose shaping with a filler, hoping to give a straight un ski like look. Can you advise and recommend a surgeon?,
A. Thank you very much for your enquiry, it’s certainly does sound as if you have had slightly reduction in the bridge of your nose and dermal fillers is certainly an option for you. Many plastic surgeons do perform dermal filler injections to the nose and you’ll be able to find a list of surgeon by looking at the BAAPS website. This website lists surgeons in your areas who has specialized in rhinoplasty procedures. I do wish you the best of luck if you do decide to go ahead with consultation. Best Wishes, Yours sincerely Prashant Govilkar Plastic and Cosmetic Surgeon Consultant, Aurora Clinics

Q. hi, My nose is not to big but is slightly on the long & pointy side, i would like to shorten it but im worried that doing this may make my nostrils appear bigger and longer?,
A. Thank you for your enquiry. It is a common impression that shortening of the nose automatically means increased visibility of the nostrils. This however does not hold true, in particular for the open tip rhinoplasty approach. If you would like to have a look at my website at the rhinoplasty outcomes gallery: http://www.allaboutcosmeticsurgery.co.uk/gallery/rhinoplasty, you will notice that the images labelled 26, 18, 14, 12, and 73 show significant reduction of the nasal length but without increased visibility of the nostrils from the front view and without elongation of the nostrils.

Q. I wondered if you may be able to advise me about whether i would benefit from rhinoplasty. I have a slight hook to my nose and a low tip, on the tip itself the cartilage has dropped slightly lower on the left side than the right. The cartilage moves up and down when you press it and i feel the difference is quite obvious and unsightly. Thank you for your time, I hope to hear from you,
A. Thank you for your enquiry. From your description, I would say that the nasal tip requires stronger support in order to provide a balanced relationship with the bridge of the nose. This can be achieved most frequently through an open rhinoplasty approach. It would be possible to evaluate using a computer simulation, what it is that you would feel comfortable with, and to establish a treatment plan able to achieve that type of change.

Q. Hey. I have a large nose but its not to big i like how it is but my left side of my nostril is a bit wider than my right and on my left nose there is a little tiny bump barely noticable. I was wondering what kind of procedure would i have to go threw would i need surgery? are could it be non-surgical i just want to make my left side look like my right,
A. Thank you for your enquiry. Based on your description, it doesn’t appear to be worth while doing any surgical intervention, as the changes anticipated are very small. In addition, the it is important to appreciate that cosmetic surgery treatments will not yield to provide you with perfect symmetry of the two sides of the nose, since there are too many little differences between our right and left sides. It is always possible to have a consultation that includes computer based imaging and simulation, which can help decide regarding the course of action and the optimal type of treatment. Nonsurgical treatments involve using injectable fillers to build up a stronger contour in areas that are deficient, and would not be a will to reduce any part of the nose.

Q. Hello, I had an open rhinoplasty 9 months ago and I am very unhappy with the results and I am definately considering a revision. i know that this kind of procedure is very diffeicult and require specific expertise. I was wondering if you would be able to advise me on finding the best surgeon for the job.,
A. Thank you for your question. I think at this stage it is very important that you try and build a very clear idea of what you would like to see as an end result. This can be assisted during the consultation by computer imaging, and clarity in terms of the surgical plan is very relevant for the end result. In terms of finding the surgeon, I suggest that you try and see several people, in particular if from their websites you have found that the pattern of work they do corresponds to your general direction for change. There is no hard and fast rule regarding finding a surgeon most suitable for you, but trying to contact and discuss your problems with several people gives you a broader appreciation of the subject and approaches. In addition, you can always ask if you can discuss with patients that had surgery with the same person, if possible with a reasonably similar scenario.
