I have a nose, which I feel is too big for my face, and it has a slight bump on the bridge

Q. I have a nose, which I feel is too big for my face, and it has a slight bump on the bridge. I would like to make my nose look smaller along with my nostrils. Is this possible?,

A. Thank you for your enquiry. In general terms, reduction rhinoplasty is one of the common types of intervention, and it may include all the elements that you describe. At the time of the consultation, using computer imaging and simulation it is possible to plan for a sensible group of changes and discuss what is achievable. Inevitably, the outcome of surgery will be somewhat different from the planning, but the preparation will facilitate increased accuracy. You can find out more regarding reduction rhinoplasty on my website at: http://www.allaboutcosmeticsurgery.co.uk/rhinoplasty/types/reduction-rhinoplasty

for as long as i can remember my nose has bothered me, it is large and has a hump on the bridge

Q. Hello, i wanted to know if you could help me, im 18 and for as long as i can remember my nose has bothered me, it is large and has a hump on the bridge, also when i laugh or smile the tip goes downward and it just does not look like it should be on my face, it is really upsetting me, and it is getting to the point where i cannot look at myself without wanting to become upset, i am so desperate to have rhinoplasty surgery, i have been reading up and learning about the procedure for many years now, but i just cannot afford it at the moment, and i doubt i will be able to afford it for a very long time, do you know anything that i could do or look into as i am at my whits end, thank you so much.,

A. Thank you for your enquiry. As a starting point, I would suggest discussing your concerns with your GP. This may be suitable for NHS treatment although it seems rather unlikely. The other point is related to the option of having the surgery through one of the television programmes or as part of a course on rhinoplasty where surgeons may demonstrate the intervention and are therefore not charge a fee. Finally, in terms of the surgical costs, there are options for having the treatment in other countries, but I do suggest that you do a fair bit of background checking before you commit.

I wondered if you may be able to advise me about whether i would benefit from rhinoplasty

Q. I wondered if you may be able to advise me about whether i would benefit from rhinoplasty. I have a slight hook to my nose and a low tip, on the tip itself the cartilage has dropped slightly lower on the left side than the right. The cartilage moves up and down when you press it and i feel the difference is quite obvious and unsightly. Thank you for your time, I hope to hear from you, joanne,

A. Thank you for your enquiry. From your description, I would say that the nasal tip requires stronger support in order to provide a balanced relationship with the bridge of the nose. This can be achieved most frequently through an open rhinoplasty approach. It would be possible to evaluate using a computer simulation, what it is that you would feel comfortable with, and to establish a treatment plan able to achieve that type of change.

I have wanted a nose job for as long as I can remember

Q. I have wanted a nose job for as long as I can remember, I have never had any direct bullying from it or anything, apart from the odd comment, its more my insecurity. But I am only just 19, how old would you recommend I be to undergo the procedure, as I realise I might be too young. Thanks.,

A. Thank you for your enquiry. I typically advise teenagers to wait until they are at least 18 before they consider having rhinoplasty surgery. Beyond this age, apart from the fact that the nasal development has more or less stabilised, the facial proportions have also entered a slower rate of change.

I had a septo-rhinoplasty done privately under insurance for medical reasons in July 2009

Q. Dear Mr Ion, I had a septo-rhinoplasty done privately under insurance for medical reasons in July 2009. This was around 18 months ago now but as a result of the operation 1. My nostrils are very noticeably different sizes and shapes now, they where not before sugery. Does not look very good. 2. By straightening my nose (which was bent to the left) the bump I had, which is still there, now has more a dent before the end of the nose where it was broken during surgery and now creates a bad looking shadow when standing under any light Is it possible to correct these with a second surgery – and if performed privately under insurance for mediacal reasons, but the overall look once finished makes me now very conscious and depressed can I follow this up with the insurance company, GP or private consultant? Also is correctove rhinoplasty more expensive than first time surgery? Thanks for your time,

A. Thank you very enquiry. I think in the first instance you should discuss this with your surgeon. I doubt that the insurance company will provide cover for the cosmetic revision, and also with regards to the NHS, I do not know whether this would be covered. You could discuss this with your GP. I cannot speak for other surgeons, but my fees for first-time or secondary rhinoplasty procedures are no different.

There are seven aspects to my nose which I am not satisfied with

Q. Plan to have rhinoplasty. There are seven aspects to my nose which I am not satisfied with – the prevalence of the tip, the seemingly permanent redness of my nose, its size, it’s shape, the size of the nostrils, the hump/bump on the nose and its lack of ‘clean and clear connection with my forehead’ – which I believe is called the nose bridge. Realistically, can I expect these problems to be corrected/improved in one operation?,

A. Thank you for your enquiry. The redness of the nose is not improved by surgery, and sometimes can be exacerbated. The typical treatment involves either intense pulsed light or laser. In the case of rosacea, a more structured dermatology treatment is required. With regards to the transition between the forehead and the nose, or the radix area, it is possible to deepen it, but there are some limitations regarding the frontal sinus, and this can be evaluated on an x-ray. Otherwise the changes that you describe are commonly part of rhinoplasty surgery and would typically be addressed in the same operation.
