About 2 years ago I had open rhinoplasty surgery which I am not happy with

Q. I am a 22 year old female. About 2 years ago I had open rhinoplasty surgery which I am not happy with. If anything I think I am more self conscious now! Is it possible to make subtle amendments, (particularly correcting the tip of the nose which point up and my nostrils looked flared? Please let me know what experience you have of secondary rhinoplasty?,
 
A. Thank you for your enquiry. Open rhinoplasty’s do take longer to settle down than a close rhinoplasty’s as a residual swelling and numbness of the skin of the nose plus can persist for up to 6 to 8 months after an open rhinoplasty. You say that it is now 2 years since you had an open rhinoplasty so I’m quite sure that the effects of the open rhinoplasty would have settle down by now.Subtle adjustments can be made to the tip of the nose but this again would need open rhinoplasty as you need to be very clear in visualization of the adjustments that need to be done on the tip of the nose .This may need ex channels surgery in the form of alar base reduction that you would entail putting a scar on the base of the tip of the nose. You would need a detailed consultation with a plastic surgeon who is undertaking rhinoplasty and regular bases. Q. I have had rhinoplasty before in 2001. I am not happy with the results and would like some more detailed advice on revision rhinoplasty. Do you perform revision rhinoplasty regularly? If so how often? Is it a successful procedure?,
 
A. Thank you for your enquiry. A revision rhinoplasty is a more demanding procedure than a primary rhinoplasty. The surgeon has to contend with the effects of a scarring from previous surgery as well as unsatisfied patient who is not happy with the results of the primary rhinoplasty. It is absolutely essential that you have a detailed consultation with a surgeon, other plastic surgeon or ENT surgeon who is qualified in udertaking primary and secondary rhinoplasty’s. Yes I do perform revision rhinoplasty regularly and would on an average I do 5- 8 revision rhinoplasty’s every year.I would say that revision rhinoplasty is a successful procedure but it would depend on what the patient expects to achieve after a secondary rhinoplasty and it is for this reason that a detailed consultation is absolutely essential. I had rhinoplasty through Transform in 2007 to correct a hump on the bridge of my nose

Q. I had rhinoplasty in 2007 to correct a hump on the bridge of my nose – the surgeon told me that I should have the tip lifted also, despite the fact I was fine with the way it looked before. At first things looked great but after the swelling went down, the tip of my nose has a huge cleff in it (one side of the cleff sticks out further than the other) and from the front in photos it looks like the bridge has been smashed, there is no real definition to it – it’s very wide and the hump seems to have been moved off to one side of my nose rather than removed. At my follow up appointment the surgeon told me after the surgery that I had a ‘difficult nose’ and he would not feel confident about correcting it. Is there anything that can be done? As this is would be revision rhinoplasty I really want to get it right – I also worry about the cost.,
 
A. Thank you for your question. Whilst it is possible to to improve on the appearance of the nose, one of the critical aspects at this stage is some accurate planning regarding what you would like to see as an end result. In some instances also, smaller adjustments, sometimes nonsurgical can bring sufficient benefits for patients to feel comfortable. With this in mind, I would suggest a consultation using computer based simulation either on standard photographs or 3-D imaging, to help you determine what it is you would like to achieve and communicate it clearly with me. It would be possible then to give you guidance regarding how much change can be operated safely, and what type of intervention may be best suited for the purpose. It is also advisable to see more than one surgeon to allow you to get the overview regarding the current situation and options for change.

I am considering a revision to reduce the size of my nostrils

Q. I am considering a revision to reduce the size of my nostrils. Is this a complicated procedure and do I have to wait 6-12 mths as this seem to bge the way for the other clinics I have contacted? Many thanks for your help,
 
A. Thank you for your enquiry. If your interest is purely to reduce the width of the nostrils, a period of six months after your operation may be sufficient. If there is significant swelling in the tip of the nose, it would be better to wait a little bit longer so that the decision regarding the extent of the changes is based on a more stable shape. The procedure may be straightforward or slightly more complicated depending on the shape of the nostrils which can vary extensively. In some instances a more significant alteration in the shape of the nostril and base of the nostril can require some cartilage grafting for support, but in most cases it is just a matter of removing some tissue at the base of the nostrils, restoring the contour and closing the wound.

7 years ago i had a rhinoplasty. it went wrong

Q. hi. 7 years ago i had a rhinoplasty. it went wrong. it was a close operation so no scar but the tip is messed up now. the surgeon has taken too much away from one nostril too much and it looks like a bone is protruding out of my tip. wnder if non surgical rhino can fix it,

A. Thank you for your enquiry. Rhinoplasty procedures vary significantly with regards to do amount of work carried out inside the nose. As far as my practice is concerned, I use silicone nasal splints inside the nose rather than nasal packs, and their purpose is to prevent contact between the lateral walls and the nasal septum whilst the swelling is present. This prevents formation of scar tissue between the septum and lateral walls (most frequently with the inferior turbinates) also known as synechiae.

I am looking at having revision surgery and have consulted with 2 surgeons so far

Q. I am looking at having revision surgery and have consulted with 2 surgeons so far. As far as the 2 surgeons go they were ok but their fees were overly cheap, which concerns me. I know I should not choose a surgeon based on their fees but im not exactly flushed at the moment either. The revison surgery I need is just on the nose tip and I wanted to know where I can view any previous work and your prices plase.,
 
A. Revision rhinoplasty surgery is very unique, so although I can show you pictures of some of my patients who have had rhinoplasties before, I probably don’t have a picture of a revision that would be similar to your nose. Nevertheless I can show you examples of some of my work but beyond that, if you’re looking for a particular outcome or several before and after photos for me to show you, that may not be possible. For revision work, it’s a lot harder to do, as well, as there’s more scarring and a greater degree of unpredictability in the final results. I think you are right to proceed with caution, but at some point you may have to decide what you think about all your consultations and go with the surgeon you feel most comfortable with.
